TERMS AND CONSENT DOCUMENT FOR PARENT/ LEGAL GUARDIAN

Welcome to my private practice. This document contains important information about my professional services and policies. Please read it carefully. When you sign this document, it will represent an agreement between us.

Length and Frequency of Visits:

The standard length of a session is 45 minutes. Frequency of sessions is arranged according to the needs and the unique situation of each client.

Being Late for a Session:

Should the client arrive late for the appointment, the scheduled visit may be shortened by the amount of time they came in late. When they arrive, we’ll make use of what time is available, though I may not be able to extend the session time.

Payment:

Parents/Legal Guardians are expected to pay the standard fee at each session unless other arrangements have been made in advance. I do not offer refund of any payment made by the client. Failure to make two payments in a row will result in suspension of services until the balance is paid in full.

Cancellation Policy & Missed Appointments:

Because an appointment time is especially reserved for the client, I require an advance notice of at least 24 hours for cancellations or changes of scheduled appointments. If an appointment is cancelled with less than 24 hours’ notice, or a failure to show up, the client will be charged for the appointment. Unusual exceptions shall be considered on case to case basis.

Confidentiality:

Confidentiality is of utmost importance whilst sensitively dealing with all clients/families. However, disclosure of such confidential information would be mandatory in the event of abuse (verbal, physical, psychological, or sexual), if the client would be at risk to harm himself/herself or others.

Contacting Me: 

Often, I’m not available immediately by telephone and will not answer the phone when I am with a client. I do not have specified call-in hours. When I am unavailable, you may leave me a text message, email or a brief message on my voice mail which I monitor frequently. Please don’t use Whatsapp. I will make every effort to return your call on the same day you make it, with the exception of weekends and holidays, unless you specify that it’s an emergency.
